2009 Benefits Overview
Full-Time Regular (REG) Employees

401(k) Retirement Plan
Long-term savings and retirement program features company matching dollars ($1 for $1 on the first 3% and 50 cents/$1 on the next 2% of employee contributions). Choice of 16 investment funds. Choice of pre-tax and/or after-tax contributions, up to 25% of savings through payroll deduction. The savings plan features loans and other transactions that can be performed online or via a toll-free telephone number. The Company will make a Retirement Contribution on behalf of each participant in the Plan (other than participants who are Hourly Employees) in the amount of 4% of the participant’s Compensation. The Retirement Contribution will be contributed on a per payroll basis.

Health/Prescription
Comprehensive health and prescription coverage that protects you in the event of an illness or injury and allows you to focus on prevention. You may enroll your spouse/domestic partner (DP)*, your unmarried child(ren)/DP child(ren) up to age 19, your unmarried child(ren)/DP child(ren) up to age 25 if a full-time student.
You may choose from one of four in-network options:
     • Option 1: $0 deductible with 100% plan coverage
     • Option 2: $300 deductible with 90% plan coverage
     • Option 3: $750 deductible with 80% plan coverage
     • Option 4: $1,000 deductible with 80% plan coverage
Employee contributions and co-payments vary based on the option selected. Special features of the plan include preventive wellness care paid at 100%, retail and mail order prescriptions, discounts on a variety of alternative therapies such as acupuncture, massage therapy, nutrition counseling, etc. and wellness services including weight loss assistance programs, fitness club memberships, hearing care services, and more.

Vision
Plan covers eye exams and materials for you and your eligible family members. Higher benefits are paid when care is obtained from network providers. Plan features include exams, prescription glasses, and contact lens care. In addition, the plan also offers discounts on Lasik surgeries.

Dental
Comprehensive dental plan offered. Plan features a $25 individual deductible/$50 family deductible and a $2,000 annual benefit maximum per individual. Coverage includes diagnostic and preventive services, basic and major dental care and varies based on level of care. Orthodontic benefits offered $2,000 lifetime maximum for eligible dependent children up to age 19.

International Expatriate Healthcare
Comprehensive plan that offers medical, dental, vision, employee assistance, and medical evacuation/repatriation. Medical plan features 80%/20% coverage with no deductible. Eye exams 100% coinsurance for eyewear subject to $100 maximum every 24 months, no deductible. Dental plan features a $25 Calendar Year Deductible and a $1,500 annual benefit maximum per individual. Coverage includes preventive, basic and major dental care and varies based on level of care. Orthodontic benefits offered $1,500 lifetime maximum for eligible dependent children under age 19. International EAP Assist with telephonic counseling. Comprehensive medical evacuation/repatriation program for you and your eligible dependents.

Flexible Spending Accounts
You may participate in one or both of the flexible spending accounts (FSA):

• Health Care Spending Account allows you to pay eligible expenses that are not covered or fully reimbursed by your medical, dental and vision coverage (for example: deductibles and co-pays) with tax-free dollars. $5,000 maximum.

• Dependent Care Spending Account reimburses you for eligible dependent care expenses that enable you (and your spouse) to work (for example: child care expenses). $5,000 maximum for single employees or those married filing jointly; $2,500 maximum for married employees filing separately.

When you sign up for these accounts, you will be able to reimburse yourself for eligible health and dependent care expenses using your tax-free dollars. This means that the out-of-pocket cost to you for these expenses will be reduced by the amount of tax that would have been deducted from your gross salary. Each person will receive 2 debit cards.

The Health Care Spending Account offers a 2-1/2 month grace period to incur claims after the end of the plan year and be reimbursed from your prior year FSA. Funds not requested will be forfeited. In other words: You use it or lose it after the grace period.

Disability Insurance
Short Term Disability: In the event of an extended absence from work for a non-occupational accident or illness that is not covered by any workers’ compensation law or act, you may be eligible for short-term disability. Basic Short Term Disability coverage is provided by AREVA. Separate sick day policy for the first seven days; then 12 weeks at 100%; 14 weeks at 60%, up to max of $12,500 per month.

Long Term Disability (LTD): For disabilities continuing beyond 6 months. Benefit of 60% of base earnings, reduced by Social Security. Basic LTD coverage is free to employees or employees may elect to pay the premium in order to receive tax-free benefit. If you elect to pay for this basic LTD insurance, you may also purchase an additional 6-2/3 percent of your current annual salary to equal a total of 66-2/3 percent of your current annual salary. Family members are not eligible for LTD coverage.

Group Life Insurance
Company-paid basic life insurance coverage of two times your annual salary, to a maximum of $400,000 or you may choose a flat $50,000 of coverage. You may purchase supplemental life insurance coverage from one to four times your annual base salary with a maximum of $750,000. These life insurance premiums are made through payroll deduction. Portable at termination.

Dependent Life Insurance
Voluntary, employee-paid insurance for spouse and/or dependent children to age 19 or age 25 if a full-time student. Spouse coverage is $25,000 or 50% of your supplemental life insurance up to $375,000. An amount over $25,000 requires spousal evidence of insurability. Dependent child(ren) coverage is $10,000. Guarantee issue to eligible new hires. Portable at termination.

Accidental Death and Dismemberment Insurance (AD&D)
Company paid basic AD&D coverage of three times your annual base salary, to a maximum of $300,000. You may also purchase supplemental AD&D coverage for you and your family. Supplemental AD&D premiums are made through payroll deduction. Portable at termination.

Business Travel Accident Insurance
Company-paid benefit. Provides term life insurance to employees while on business travel. Amount of coverage varies by exempt-non-exempt status and salary level.

Long Term Care Insurance
Voluntary, employee-paid insurance that provides for personal care or supervision needed by persons of all ages for an extended period of time. Choice of three levels of daily maximum benefits at $115, $205, or $290. Bank drafting. Guarantee issue to new hires in salaried positions at time of hire. Portable at termination.

Vacation
Paid vacation is based on continuous years of company service according to the following schedule. Vacation days are accrued on a monthly basis.

Holidays
The company observes 11 paid holidays each year; however, this schedule may vary within different areas based upon business considerations. New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and day after, Christmas Week. In the event that New Year’s Day or Independence Day falls on a weekend, a floater holiday will be observed.

Educational Assistance
The company encourages employees to continuously improve and develop their knowledge and skills through education. 100% of tuition will be reimbursed for approved courses, up to a maximum of $8,000 per calendar year. $5,250 provided tax free and remainder is taxable. An overall cumulative 2.5 GPA is required.

Employee Assistance Program (EAP)
The EAP is available to you and your family members at no cost, 24 hours a day, 7 days a week. This program provides personal family, financial, and mental health counseling and referral services to employees and family members on a confidential basis. Company paid.

*Domestic partners are eligible for health, dental and vision coverage.
